Melatonin is the most popular sleep supplement in the world. In the United States alone, usage has increased more than fivefold over the past two decades. You can find it in gummies, capsules, tablets, liquid drops, and even children's chewables at virtually any grocery store or pharmacy.
And yet, most people who take melatonin nightly know very little about what it actually does, how much they should be taking, or what the side effects look like over time. This is not an anti-melatonin article. Melatonin has real, evidence-backed uses. But the gap between how people use it and what the science actually supports is wider than most realize.
What Melatonin Actually Does (and Does Not Do)
Melatonin is a hormone produced naturally by the pineal gland in your brain. Its primary job is not to make you sleep. Instead, it signals to your body that darkness has arrived and it is time to prepare for sleep. Think of melatonin as a sunset signal rather than a sleeping pill.
Your body typically begins producing melatonin in the evening as light levels drop, with levels peaking between 2:00 and 4:00 a.m. and declining toward morning. The average adult produces between 0.1 mg and 0.9 mg of melatonin per day. This is important context for understanding why the doses sold over the counter are so problematic.
Because melatonin primarily affects the timing of sleep rather than the depth or duration of sleep, it works best for circadian rhythm issues: jet lag, shift work adjustment, and delayed sleep phase disorder (when your internal clock runs late). It is far less effective for the most common complaint people have: waking up at 3 a.m. and not being able to fall back asleep.
The Common Side Effects Most People Dismiss
Because melatonin is sold as a supplement and marketed as "natural," many people assume it has no side effects. That assumption is wrong.
The most frequently reported side effects include headaches, daytime drowsiness, dizziness, nausea, and vivid dreams or nightmares. Some people also experience irritability, short-term feelings of depression, stomach cramps, and reduced alertness the following morning.
Morning grogginess is particularly common and particularly counterproductive. If you are taking melatonin to sleep better but waking up foggy and slow, the supplement may be doing more harm than good. This effect often stems from taking too high a dose, taking it too late at night, or both.
Less commonly discussed are the hormonal effects. Melatonin interacts with the reproductive system, and some studies have noted changes in levels of estrogen, progesterone, and luteinizing hormone with regular supplementation. For most adults, these effects appear minor at typical doses, but they are worth knowing about.
The Dosing Problem Nobody Talks About
This is where the story gets uncomfortable. The average melatonin gummy or tablet sold over the counter contains between 3 mg and 10 mg of melatonin. Some products go as high as 20 mg. Remember that the human body produces between 0.1 mg and 0.9 mg per day. That means a standard 5 mg supplement delivers roughly 10 to 50 times what your body produces naturally.
Research from MIT found that a physiological dose of just 0.3 mg effectively restored sleep efficiency and raised plasma melatonin to normal nighttime levels. The same study found that a 3 mg dose (ten times higher) also improved sleep onset but caused hypothermia and kept melatonin levels elevated well into daylight hours. Higher doses did not produce better sleep outcomes. In fact, some research suggests that supraphysiological doses (those exceeding 5 mg) may actually disrupt the melatonin phase-response curve, making your circadian rhythm less stable over time.
A Cochrane review confirmed that doses above 5 mg appear no more effective than lower doses for improving sleep. The takeaway is straightforward: more is not better, and the doses most people are taking wildly exceed what the evidence supports.
What Is Actually in Your Supplement?
In the United States, melatonin is classified as a dietary supplement, not a drug. This means the FDA does not evaluate it for safety, efficacy, or labeling accuracy before it reaches shelves. There are no federal requirements that companies verify whether their pills contain the amount of melatonin stated on the label.
The consequences of this regulatory gap are significant. A 2017 study published in the Journal of Clinical Sleep Medicine tested 31 melatonin supplements and found that more than 71% did not contain the amount of melatonin listed on the label (within a 10% margin). Actual melatonin content ranged from 83% less than labeled to 478% more. Even different lots of the same product varied by as much as 465%.
Perhaps most concerning, 26% of the supplements tested contained serotonin, a neurotransmitter not listed on the label. Serotonin supplementation can cause serious side effects, especially in people taking antidepressants or other serotonergic medications.
A more recent 2023 study published in JAMA examined 25 melatonin gummy products and found that 88% were inaccurately labeled. Only three products fell within 10% of their declared melatonin quantity. If you want any assurance about what you are actually consuming, look for products bearing the USP (United States Pharmacopeia) verification mark, which indicates third-party testing for content accuracy.
Tolerance, Dependency, and the Psychological Trap
The good news: melatonin does not appear to cause physical dependence in the way that prescription sleep medications like benzodiazepines do. You will not experience classic withdrawal symptoms if you stop taking it, and most research suggests that tolerance development is minimal.
The less straightforward news: many people develop a psychological dependency on melatonin. They come to believe they cannot fall asleep without it, and that belief itself becomes a source of anxiety at bedtime. If you have been taking melatonin nightly for months or years, the thought of going without it can feel stressful, and that stress is more than enough to keep you awake. It becomes a self-fulfilling cycle.
There is also no strong evidence that exogenous melatonin permanently suppresses your body's natural production. Most studies show that natural melatonin production returns to baseline within days of stopping supplementation. But the psychological reliance can be harder to shake than any physiological effect.
Long-Term Use: What the Newest Research Shows
Long-term safety data on melatonin supplementation is limited, which is itself a concern for a product that millions of people take every night for years. Most existing studies cover only short-term use of one to three months.
A preliminary study presented at the American Heart Association's 2025 Scientific Sessions reviewed five years of health records for more than 130,000 adults with insomnia. Those who had used melatonin for at least one year had an 89% higher risk of developing heart failure, a threefold increase in heart-failure-related hospitalizations, and double the rate of all-cause mortality compared to non-users. These findings have not yet been published in a peer-reviewed journal and cannot establish cause and effect. People who take melatonin long-term may have more severe underlying sleep issues or other health conditions contributing to these outcomes. But the signal is strong enough that researchers are calling for more investigation.
Who Should Be Especially Careful
Certain groups should exercise particular caution with melatonin or avoid it entirely.
People on blood thinners. Melatonin can increase bleeding risk when combined with warfarin and other anticoagulants.
People taking immunosuppressants. Melatonin stimulates immune function, which can directly counteract immunosuppressive therapy used after organ transplants or for autoimmune conditions.
People with diabetes. Melatonin can affect blood sugar levels and may interact with diabetes medications. If you are managing blood glucose, talk to your doctor before adding melatonin.
People taking antidepressants. Given the serotonin contamination found in many supplements and melatonin's own interactions with serotonergic pathways, combining these without medical guidance carries unnecessary risk.
Children and adolescents. Melatonin use in children has grown dramatically, but long-term safety data is thin. Animal studies have raised concerns about effects on puberty-related hormones. Human studies have been mixed: some show no impact on pubertal development over two to four years, while one study noted potential delays with use exceeding seven years. The American Academy of Sleep Medicine issued a health advisory urging caution, and the American Academy of Pediatrics recommends that melatonin use in children only happen after consultation with a pediatrician and after establishing healthy sleep habits first.
When Melatonin Actually Works
None of this means melatonin is useless. The research supports its use in specific situations.
Jet lag. This is perhaps melatonin's strongest evidence-based use case. Taking a low dose (0.5 to 3 mg) close to bedtime at your destination can help reset your internal clock after crossing multiple time zones.
Delayed sleep phase disorder. If your natural sleep window is significantly later than you need it to be (say, you do not feel sleepy until 2 a.m. but need to wake at 7 a.m.), low-dose melatonin taken several hours before your desired bedtime can help shift your circadian rhythm earlier.
Shift work. Workers rotating between day and night shifts can benefit from melatonin's circadian-shifting properties to help their bodies adjust to new sleep windows.
Short-term sleep onset difficulty. A meta-analysis found that melatonin reduces sleep onset latency by an average of about seven minutes compared to placebo. That is a real but modest effect. Notably, the improvement was more significant when measured subjectively (how quickly people felt they fell asleep) than objectively (what sleep tracking devices recorded).
For general insomnia, especially the kind involving middle-of-the-night wakefulness, melatonin's benefits are limited. It is a circadian signal, not a sedative, and using it as one misses the point of what the hormone does.
Better Approaches to Sleep
If you are reaching for melatonin because you are struggling to sleep, it is worth asking whether you have addressed the more fundamental factors first. Cognitive behavioral therapy for insomnia (CBT-I) is considered the gold standard treatment for chronic insomnia by every major sleep organization. It works by addressing the thought patterns and behaviors that perpetuate poor sleep, and its effects last longer than any medication or supplement.
Temperature regulation, consistent sleep schedules, light exposure management, and reducing evening stimulation all influence sleep quality more than any supplement can. These are not glamorous interventions, but they work with your biology rather than attempting to override it.

The Bottom Line
Melatonin is not dangerous for most people when used correctly: low doses (0.3 to 1 mg), short-term, for the right conditions. The problem is that almost nobody uses it this way. Most people take doses that are 10 to 50 times what their body produces, every night, for months or years, for a type of insomnia that melatonin was never designed to address.
